Online enrollment is only available to Group A   PDF Document (Reader Required; Click to Download) members over age 18.
We offer personal and confidential service whether you enroll online or through our toll-free hotline. Telephone enrollment is available to Group A and Group B members.

July 7, 2010 is the last day to begin enrollment. This means that you must call and begin the enrollment process by this date.
How It Works
Online
To get started online, Group A   PDF Document (Reader Required; Click to Download) members click here. A benefit coordinator will call you to complete your enrollment once you have submitted all required information.
By Telephone
To enroll by phone, please call the toll-free number above. Your call is confidential. Benefit coordinators are available to help you decide which services are right for you and your family, and to help you find a New York licensed provider.
If you are applying for someone else:
If your patient is interested in enrolling, or you are calling for a family member or friend, we encourage the person to call the toll-free number above to enroll. If you are enrolling a patient, family member or friend, please make sure you have their consent.
